Transfer of care around medicines (TCAM)

Patients who would benefit from extra support with their prescribed medication following discharge from hospital are the focus of the Transfer of Care Around Medicines (TCAM) initiative.

The service went live in Buckinghamshire in December 2019. Patients discharged from Buckinghamshire Healthcare NHS Trust are identified by hospital pharmacists and, with their consent, referred to a community pharmacist of their choice for advice and to discuss any queries about their medicines.

This approach is already improving communication between hospital and community pharmacists and will lead to improvements in the quality, safety, effectiveness and timeliness of patient care.
